Not sure the top photo shows a wardrobe malfunction. The young actress appears to have metal caps on her breasts. As no nipple seems to be visible this is technically OK.

In an installment of Playboy's Sex in the Cinema series there is a shot of Daniela Rocca full front and naked from the waist up in Esther and the King. Apparently the scene was shot both ways but only the concealed version was used.
